2011-2012 Forms for returning families

 All forms are due on the last day of school, June 8 by 12:30.

Hello returning SREC parents,
 
Mark Your Calendars

• There will be a Work Party on Saturday, August 13 from 9am to 2pm. Please plan on attending to help prepare classrooms for the new school year.
• First Monday Night Meeting is Monday, August 22 from 6:30pm to 8:00pm
• School starts on Wednesday, August 17 at 8:15am until 12:15pm for elementary and 12:30 for middle school. 

Wednesday is the ONLY minimum day that first week. 
 
Necessary Forms To Start School
We have forms on file for most of you, so please fill out only the following:
 

• Emergency and Identification Form   Please complete BOTH sides.  YOUR CHILD CANNOT ATTEND SCHOOL EVEN FOR ONE DAY WITHOUT THIS FORM COMPLETED. WE MUST HAVE A NEW FORM EVERY YEAR. 
• Negative TB Test Results form for all parents who will be working in the classroom. Adults will not be allowed contact with the students without this form on file. TB tests expire every four years for persons working directly with children, so keep in mind you may need a new test.
• Field Trip Driver Form and Proof of Insurance. If you plan on driving on any field trip, you will need to complete this form, provide a copy of your current insurance card, and your driver’s license. Please remember to give us a copy of your new insurance card when you renew your insurance.
• Demographic Information Form. This form helps us to obtain grant money and get an idea about the demographic make-up of the school community. All information is completely confidential.

 
Before and after school hours: Students are to be either in the Children’s Center, in adult monitored after-school programs, or off the campus when school is not in session.   Supervision is provided only 15 minutes prior to the start of school and 15 minutes following the end of the school day.
